GUY’S AIRWAY ADVANCED TRAINING MODULE:
 
Educational Supervisor: Dr Imran Ahmad

Aims:

  1. Experience in managing anticipated and unanticipated difficult airways
  2. Experience in advanced airway techniques
  3. To be confident in performing an awake fibreoptic intubation (AFOI) solo
  4. Experience in running an airway list
  5. Experience in producing written work for publication and submission to major anaesthetic meetings
  6. Teaching experience on advanced airway courses
  7. Experience in organizing airway workshops
 
Theatre Lists:
 
Every week in Guy’s main theatres there are:
  • 3 airway lists on Mon/Tues/Weds (Th 1,2 & 3)
  • 2 airway lists on Thurs (Th 2 & 3)
The majority of work will be in Ear Nose & Throat and Maxillofacial surgery – oral, pharyngeal and laryngeal surgery, neck surgery including thyroid and mediastinal work and facial surgery. Both specialities include complex cases often combined with plastic surgery. Occasionally you may be assigned to a dental list and day surgery ENT lists.

Clinical experience:
By the end of the fellowship the trainee should be competent in all of the following:

  1. Fibreoptic intubations- aim for around 20-30 AFOI over the 6 months
  2. Use of videolaryngoscopes (Airtraq, APA, CMAC, Kingvision, Glidescope)
  3. Intubation through SGAs
  4. Scalpel cricothyroidotomy
  5. Cricothyroid puncture, fine bore and wide bore cannulae
  6. Surgical airway techniques, including performing a surgical tracheostomy
  7. Management of supraglottic & subglottic lesions
  8. High Frequency jet ventilation, with the Monsoon ventilator and Manujet
  9. THRIVE/apnoeic oxygenation techniques
  10. Extubation strategy in patients with anticipated difficult airways
  11. Running an airway list and communication with surgical colleagues
  12. Complete a free flap case, transfer and handover to GCCU

Currently this fellowship is only open to Anaesthesia South London (ASL) trainees
